Privacy policy at Pentabase A/S
Our privacy policy described hereafter presents procedures on how we process data and comply with the EU General Data Protection Regulation 2016/679 on the protection of personal data when using our service and tells you about your privacy rights and how the law protects you. The EU Regulation 2016/679 concerns the processing of personal data and the free movement of such data as well as any other applicable legislation regarding data protection.
We will retain your personal data only for as long as is necessary for the purposes set out in this Privacy Policy. We will retain and use Your personal data to the extent necessary to comply with our legal obligations (for example, if we are required to retain your data to comply with applicable laws), resolve disputes, and enforce our legal agreements and policies.
We are legally obliged to maintain patient record-keeping in Danish jurisdiction. We may submit your data to entities operating under the Danish Ministry of Health in Denmark for public statistics, COVID-19 tracking, and is made accessible via the Danish Health Portal, www.sundhed.dk and to your doctor.
Information we collect
While using our service, we may ask you to provide us with certain personally identifiable information that can be used to offer you our services. Personally, identifiable information may include but is not limited to name, e-mail address, telephone number, passport number and personal ID number (CPR-number). We collect and processes your data when you do the following;

• Send us questions or feedback
The data controller and processor
PentaBase A/S is the data controller and processor for collecting, processing and usage personal data collected through journal.pentabase.com. The purpose of processing the collected data is to provide our analytical services to our customers and to provide support for troubleshooting at the same. Pentabase A/s is registered at Petersmindevej 1A, 5000 Odense C, Danmark, info@PentaBase.com.
Basis of processing and purpose
We collect contact information such as name, date of birth and e-mail address to deliver the service requested and to send test results. We may also share your information with the relevant authorities under the Danish Ministry of Health to track COVID-19 positives and general data on test activities as required by current Danish legislation and research.
Your data will be deleted five (5) years after you receive your service.
Third-party recipients of personal data
We do not sell, transmit, or transfer your data to third parties or third countries.
The Data Processor hosts journal.pentabase on enavn.dk, which acts as a sub-data processor to PentaBase Aps. All the data you provide on the journal are stored on servers of the sub-data processor, IT Forum.
We use an external company as a supplier to deliver our services in the best possible way. The external supplier is a data processor and processes personal data, in certain cases, in connection with their provision of services to us. Our sub-data processor only processes personal data according to our instructions and per the data processing requirements of the legislation.
We have entered into a data processing agreement with our data processor, which is our guarantee that they comply with applicable regulations regarding the protection of your personal data.
Your rights
As the registered person, you have several rights that we must comply with and ensure at all times. You have the right to request the following;
• Update or correct your personally identifiable information
• Delete the personally identifiable information maintained about you on our systems (subject to the following paragraph), by cancelling your account. Such updates,
corrections, changes and deletions will have no effect on other information that we maintain or information that we have provided to third parties in accordance with this privacy policy prior to such update, correction, change or deletion. To protect your privacy and security, we may take reasonable steps (such as requesting a unique password) to verify your identity before granting you profile access or making corrections. You are responsible for maintaining the secrecy of your unique password and account information at all times. If you no longer wish us to process your personal data or to restrict the processing of your personal data, you can also send us a request to our e-mail address info@pentabase.com
• Upon acceptance, you declare that the self-sampling of the saliva sample has been performed in accordance with the applicable guidelines/instructions from PentaBase and that the sample material (saliva) belongs to the person whose personal data is linked to this profile.
Test results
A notification is sent via SMS or e-mail to the phone number or mail that you have provided when your results are ready.
